A validation link has been sent to your email account.  If you do not receive this email in your inbox within the next few minutes, please check your "junk/spam" mail.  If you do not receive the validation link email at all, please contact us directly on the email address provided below.

It is now time to complete the full application. 

NB!!  Before you continue though, we ask that you please read all the information provided below as it is vital to ensure that you are making an informed decision when you sign up with us.

1. Take your time when completing the application as accuracy and honesty are paramount (e.g. your/ your family's medical history).
2. Please answer ALL the questions and provide as much information as possible.  The more information you provide for the recipient to make their decision, the better your chances of a successful match.
3. Please ensure that when you download your photos, that you include 8 recent, hi-resolution photographs of yourself, of which 1 must be a close up facial shot and another a full length shot, with your profile.  Photos must be of you and not group photos.  Try to ensure that they are natural and feminine (i.e. no pouting or seductive poses).
4. We also require a scanned copy of your ID and a valid passport (Your passport must have a 9 month validity, in other words, it will not be expiring within the next 9 months).
5. Your donor profile will be visible on the password protected Fertility Mate website. Rest assured that NO personal details are visible!

Please remember that you have 30 days to complete your full application online.  If you do not complete it within the given time, it will sadly be moved to file 13.
